The Case Manager RN coordinates the overall interdisciplinary plan of care for patient, from admission to discharge. The purpose of this position is to provide ongoing support and expertise through comprehensive assessment, planning, implementation and overall evaluation of individual patient needs. The overall goal of the position is to enhance the quality of patient management and satisfaction. Promotes continuity of care and cost effectiveness, through the integration of case management, utilization review, and discharge planning. This position is accountable for the care, and coordination of all patients.
Essential Job Functions
•Provide case management/utilization review and discharge planning to ensure that the patient progresses through the continuum of care and is discharged to the most appropriate and least restrictive environment.
•Coordinate the integration of social service function into patient care
•Acts as patient advocate: investigates and reports adverse occurrences and performs staff education related to resource utilization, discharge planning and psychosocial aspects of healthcare delivery
•Complete expanded assessment of patient and family needs at time of admission
•Collaborate with clinical staff in the development and execution of the plan of care and achievement of goals
•Monitor the care plan daily to ensure the effectiveness and appropriateness of in? patient services; ensure that services are being delivered and meet the needs of the client upon discharge
•Assess the physical, functional, social, psychological, environmental, and financial needs of clients/patients; identify a comprehensive plan to meet the families’ service needs, and implement the plan.
•Mentor interdisciplinary healthcare team in critical thinking processes essential to continuum of care issues.
•Coordinates patient and family education as needed.
•Active Commission for Case Management Certification (CCMC) desired

•Proven team building skills in dealing within intra-professional clinical and operations teams including medical staff in diverse settings
•Ability to establish and maintain collaborative and effective working relationships
•Ability to communicate professionally and effectively in oral, written and electronic formats
