The Licensed Social Worker (LSW), as part of an interdisciplinary team, including the patient/family, physicians, nurses, advanced practitioners and community health assistants, ensures that the team is assessing, supporting and intervening on behavioral and social issues that interfere with optimal outcomes. Provides input into the plan of care and works with patients with significant behavioral, substance abuse or social issues.
Interacts with patients, family members, healthcare professionals, and community and state agencies and serves as a liaison between the patient/family and community agencies / hospitals or other services for the exchange of clinical and referral information.
Along with the Geisinger at Home team, identifies high risk patients from a psychosocial/financial perspective, assesses the psychological needs of patients and families and provides information, support, counseling, care management, and referrals to appropriate resources.
Works collaboratively with the care team to ensure patient needs are met and care delivery is coordinated across the continuum. The expertise of the LSW is sought to resolve psychosocial patient care issues and to develop and implement a complex patient transition/discharge plan for those hospitalized as well as coordination of services for those in the home. Ensures that patients are assisted to achieve their highest level of function and to stay in the least restrictive environment.
Reports to the Director, Clinical Management, Geisinger at Home.

EDUCATION AND/OR EXPERIENCE:
Master's Degree in Social Work (MSW) from accredited university required.
Care Management certification is required after two years of hire.
A minimum of two (2) years of clinical experience as Social Worker is preferred.
Experience in dealing with extended care facilities and community healthcare agencies.
A working knowledge of insurance coverage, various governmental programs, available community care agencies and long-term care facilities.
Knowledge of Social Work theories, therapies and techniques as used in individual family and group treatment.
WORKING CONDITIONS/PHYSICAL DEMANDS:
Work is typically performed in the home as well as office setting with telephonic follow-up.
