The goals of Healthy Families America Program through Delaware HOPE are to improve birth outcomes, reduce rates of child abuse, strengthen families, enhance child health and developmental outcomes, and promote family stability and economic self-sufficiency to parents with substance use disorders.
Provide intensive nurse home visiting services to women with substance use disorder and possible co-occurring mental health disorders who are receiving medication assisted treatment. Services will begin during pregnancy and after the baby is born, up until the child’s third birthday. Nurses carry a case load of 10-15 high-risk mothers and their children, and will be working in tandem with a peer recovery coach/support specialist. Following the Healthy Families America, evidence-based home visiting model, the nurse will provide support to the families in areas such as providing prenatal education and assessment, providing lactation education and support, assessing mental and physical health of the mother, assessing the child’s developmental milestones, and education on preparing and caring for an infant or toddler, while supporting the mother in her recovery.
The nurse will be provided with weekly reflective supervision sessions to promote continued growth with the program. Individuals must be compassionate nurses with previous work experience in the following areas: substance use disorder, maternal-infant health education, pediatrics and/or community-based nursing.
Strong interpersonal, verbal/written communication skills, required. Must be flexible, with the ability to work some evenings. Associates Degree in Nursing (AND or AAS) with experience in public health, community and/or maternal-child health, required. Bachelor’s Degree in Nursing (BSN) with experience in public health, community, and/or maternal-child health, required. Must have valid driver’s license, which meets the requirements of the agency’s insurance carrier and reliable transportation.
