At the full performance level of the position, the incumbent serves as a Nursing Assistant in Nursing Service at the White River Junction VA Medical Center. The Nursing Assistant will assist providers in the Covid screening area and will assist in other areas / Outpatient Specialty Clinics, as needed. It is expected that the incumbent will possess/acquire the necessary skills in the specific tasks/test/procedures to meet patient care and Service needs specific to the organizational element to which assigned. Assignments at this level include but are not limited to: interacting with both internal and external customers; obtaining medical information from patients, providing educational materials to patients, coordinating information/consults and actions related to patient care and services; takes all vital signs -weight, blood pressures, temperatures, I&Os; does routine standardized diagnostic procedures such as EKG's, Glucometry and Pulse Oximetry; collects lab specimens, processes medical records, notifies bed control for patient movements, and documents work. The incumbent will also be responsible for stocking all supplies in patient rooms and transporting patients as necessary. Demonstrated ability to take accurate vital signs including blood pressure, weight, height, pulse, pulse oximetry. Ability to perform CPS and First Aid procedures in patient emergencies. Utilize glucose monitoring machine to receive glucose readings for veterans and refer deviation to appropriate health care staff. Knowledge of medical terminology. Provide direct care to complex patients. Make observations of subtle changes in patient condition, i.e. behavior shifts, mood, skin coloring, appetite, etc., respond appropriately within the NA assigned role and contact the appropriate nursing personnel. Document care given, observations and responses on appropriate tools or forms. Administer complete tracheostomy care including suctioning. Monitor and document patient intake and output and report deviations timely and accurately. Perform nursing procedures within scope of practice and according to policies and standard procedures to care for the veteran including but not limited to enemas, catheterization, colostomy care, skin care simple dressing and application of braces and prosthetic devices. Recognize patient autonomy and promote independence by means of respect, limit setting and individualized treatment. Actively engage in treatment planning meetings. Participate in maintaining a clean and safe patient environment. Remove defective equipment, report hazards, observe safety regulations, contribute to infection control and utilize proper aseptic technique in performance of duties. Serve as preceptor and mentor to other NAs. Orient members of the health care team to units as appropriate. Manage disruptive patient behavior and appropriately intervene and request assistance from other health care members. Actively participate in the ward to which assigned, attending staff meetings and participating in performance improvement projects. Ability to work independently in the accomplishment of a wide variety of duties including setting priorities and coordinating work. Ability to communicate effectively and professionally with employees at varying grade levels and with patients and their family's various backgrounds and educational levels.
