Position Summary: This is a full-time RN/LPNlevel position responsible for conducting the Medicare Annual Wellness Visit for Medicare beneficiaries under the supervision of the patients Primary Care Provider (PCP). The ideal candidate demonstrates competency in the deliveryof care to the Medicare population, using the nursing process to assess and evaluate assigned patients and plans, implementing and documenting Annual Wellness Visit requirements. Coordinates and collaborates with multidisciplinary team members to facilitateintegrated and comprehensive health assessment and care planning related to the Medicare Annual Wellness Visit. Possesses clinical knowledge and skills to meet standards as required by ambulatory nursing. May require travel to multiple physician office practices.
Minimum Requirements: Three years of professional experience in a clinically related area preferred Demonstrated assessment, critical analysis, and written and verbal communication skills Knowledge of funding resources and clinical standards and outcomes Possess and demonstrate critical thinking skills and clinical problem-solving techniques. Possess appropriate interviewing and negotiating skills Proficient and comfortable with the use of computers and learning new computer and electronic health records systems Knowledge of managed care and population-based disease management and prevention concepts and strategies.
Education/Licenses/Certifications: Current unencumbered RN/LPN licensure in the state of Washington Basic Life Support health care provider card
